Jennifer DeGraauw, MD

Dr. Jennifer deGraauw, MD, is a Nephrologist at CommonSpirit Nephrology Durango. She is driven by a deep passion for tailoring her care to each individual patient. She prioritizes taking the time to truly get to know her patients and understand what matters most to them. With additional training in palliative care, Dr. deGraauw particularly enjoys helping patients and their families navigate illness and challenging medical decisions, profoundly appreciating the trust they place in her and her dedicated team.

Dr. deGraauw's extensive education includes her medical degree from Louisiana State University Health Sciences Center in New Orleans. She completed her Internal Medicine Residency at Oregon Health Sciences University. Further specializing, she pursued a Nephrology Fellowship at the University of Colorado and a Hospice and Palliative Care Fellowship at the University of New Mexico, providing her with a unique and compassionate approach to complex medical conditions.

Outside of her medical practice, Dr. deGraauw embraces an active outdoor lifestyle. She enjoys hiking, mountain biking, spending quality time with her family, participating in anything her kids enjoy, and fly fishing.
