  • Advanced Diagnostics: High-resolution imaging and specialized procedures for accurate, timely diagnosis.

Prostate Health & Surgical Prostatectomies

Our urologists specialize in comprehensive prostate care, from screening and early detection to advanced treatment options like surgical prostatectomy and management of post-surgical effects.

A prostatectomy is a surgical procedure to remove all or part of the prostate gland. It is primarily performed to treat prostate cancer, but can also be used in some severe cases of benign prostatic hyperplasia (BPH) when other treatments have failed.

Types of Surgical Prostatectomy

  • Radical Prostatectomy: This procedure involves removing the entire prostate gland, seminal vesicles (glands that produce semen), and sometimes nearby lymph nodes. It is a common and effective treatment for localized prostate cancer. Our surgeons are adept at performing:
  • Open Radical Prostatectomy: A traditional approach through an incision in the lower abdomen.
  • Laparoscopic Radical Prostatectomy: A minimally invasive approach using small incisions and specialized instruments.
  • Robot-assisted Laparoscopic Radical Prostatectomy: The most advanced minimally invasive technique, offering enhanced precision, magnified 3D visualization, and greater maneuverability for the surgeon. This often leads to less blood loss, less pain, shorter hospital stays, and quicker recovery times compared to open surgery.

Pelvic Floor Physical Therapy

Pelvic Floor Physical Therapy

Pelvic Floor Rehabilitation

Pelvic floor physical therapy is a cornerstone of recovery for many urological and urogynecological conditions. We collaborate closely with specialized physical therapists who understand the complex connection between the pelvic muscles and your overall health. By focusing on coordination and strength, we help you regain control and alleviate chronic discomfort.

  • Symptom Assessment: Detailed questioning about incontinence type (stress, urge, mixed, fecal), frequency, severity, triggers, and impact on quality of life.
  • Functional Assessment: Evaluation of posture, breathing patterns, movement mechanics, and core stability.
  • External Pelvic Assessment: Evaluation of perineal skin integrity, muscle tone, scarring, and posture.
  • Internal Vaginal/Rectal Examination: (With patient consent) To assess pelvic floor muscle strength, coordination, endurance, tenderness, fascial restrictions, and identify trigger points.
  • Palpation: Manual assessment of individual pelvic floor muscles.
  • Observation of Contraction/Relaxation: Assessing patient's ability to contract and relax muscles.
  • Anatomy and function of the pelvic floor muscles
  • Understanding urinary, bowel, and sexual health
  • Body mechanics and posture
  • Fluid intake and dietary irritants
  • Bladder and bowel habits
  • Pain science education
  • Pelvic Floor Muscle Training (PFMT) / Kegel Exercises:
    • Manual Biofeedback: Therapist's fingers provide feedback during internal palpation.
    • Verbal Cueing: Guided instruction on correct contraction and relaxation.
    • Targeted Strengthening & Endurance Training: Customized programs for individual muscle deficits.
    • Coordination Training: Teaching patients to engage muscles appropriately during functional tasks (e.g., coughing, lifting).
  • Biofeedback (external and internal):
    • Surface Electromyography (sEMG) Biofeedback: Uses electrodes (external or internal) to display muscle activity on a screen, allowing patients to visualize and learn to control pelvic floor muscle contractions and relaxation.
    • Pressure Biofeedback: Using a perineometer or similar device to quantify pelvic floor muscle strength.
  • Neuromuscular Electrical Stimulation (NMES):
    • Application of low-level electrical current to muscles via internal or external probes to: Improve muscle strength and endurance (for weak muscles).Decrease urgency/frequency by calming overactive bladders.Reduce pain.
  • Urge Suppression Techniques: Behavioral strategies to manage bladder urgency.
  • Bladder Retraining: Gradually increasing the time between voids to improve bladder capacity and control.
  • Bowel Management Strategies: Education on diet, fluid intake, toileting positions, and techniques to manage constipation or fecal incontinence.
  • Core Stabilization & Functional Movement Training: Integrating pelvic floor muscle activation with global core strength, posture, and body mechanics during daily activities.
  • Patient-Specific Exercise Programs: Tailored home exercise programs to maintain and progress gains.
  • Myofascial Release: Gentle, sustained pressure to release tightness and restrictions in connective tissue of the abdomen, hips, low back, and pelvic floor (internal/external).
  • Trigger Point Release: Applying pressure to hyper-irritable spots within muscles to alleviate pain and dysfunction.
  • Scar Mobilization: Gentle massage and stretching techniques to improve pliability and reduce pain/restriction from surgical scars (e.g., C-section, episiotomy, prostatectomy, hysterectomy).
  • Visceral Mobilization: Gentle manual techniques to address restrictions in the mobility of internal organs.
  • Joint Mobilization: For hip, sacroiliac (SI), and lumbar spine joints that may contribute to pelvic pain or dysfunction.
  • Dilator Therapy: (For dyspareunia, vaginismus) Gradual use of dilators to gently stretch and desensitize vaginal tissues.
  • Vaginal Trainers/Weights (Cones): For strengthening pelvic floor muscles.
  • Diaphragmatic Breathing Techniques: To promote relaxation, improve core cylinder function, and reduce tension.
  • Therapeutic Exercise: Targeting specific muscle groups (adductors, glutes, core) to support pelvic health.
  • Cold/Heat Therapy: For pain management.
  • Diastasis Recti Abdominis (DRA) Rehabilitation: Specific exercises and education to restore abdominal wall integrity.
  • Perineal Tear Management: Scar tissue mobilization and pain management after episiotomy or tearing.
  • Postpartum Return to Exercise Guidance: Safe and progressive return to physical activity.
  • Pubic Symphysis Dysfunction (PSD) and Sacroiliac Joint (SIJ) Pain Management
  • C-Section Recovery: Scar massage, abdominal bracing, and functional movement.
  • Post-Prostatectomy Rehabilitation: Pre-operative (prehab) and post-operative PFMT for urinary incontinence and erectile dysfunction after prostatectomy.
  • Chronic Pelvic Pain Syndromes (CPPS): Addressing muscular tension, trigger points, and nerve involvement.
  • Pudendal Neuralgia: Specific techniques to decompress and desensitize the pudendal nerve.
  • Erectile Dysfunction (Musculoskeletal Causes): Addressing pelvic floor muscle imbalances.
  • Fecal Incontinence: Tailored strategies for bowel control.
  • Strengthening: Targeted exercises to support the pelvic organs and improve bladder/bowel control.
  • Relaxation: Techniques to address tightness or spasms causing pelvic pain.
  • Functional Training: Coordinating pelvic muscles with daily activities to reduce symptoms and improve quality of life.
  • Collaborative Care: Our physical therapists work directly with your urologist or urogynecologist to ensure your recovery plan is integrated into your overall treatment journey.
