Home Hospice
St. Anthony Home Palliative & Hospice offers in-home and residential hospice care designed to help patients comfortably reach their end of life goals. You and your family will help plan the kind of care received and will play a vital role in communicating needs, concerns or questions to the members of the hospice team.
Palliative Care
We offer palliative care services to patients wherever they call home, which would be in their own residence, a nursing home, or other residence.
Skilled Nursing Care
Our registered nurses provide skilled care such as end of life care, wound care, medication monitoring, disease and nutrition education, and administration of I.V. medications.
Personal Care Services
Certified Nursing Aides can provide intermittent assistance with personal care needs such as bathing.
Social Work
Medical social workers are available to connect you with community resources for additional help.
Spiritual Care
Non-denominational chaplains can visit with you to provide support and education to enhance your spirituality as you strengthen your body and soul.
Volunteer
St. Anthony Home Palliative & Hospice has a strong commitment to bringing our communities the very best home hospice care. Our team demonstrates the highest level of compassion to assist our patients and families who are dealing with end-of-life issues. A very important part of that care comes from our volunteers who are the heart and soul of any hospice service. Whether our volunteers are doing clerical work, helping with supplies or visiting with a patient in the home, they are always good medicine.
